British Army WWI Pension Records 1914-1920 shows that he died 11/05/1917:
Record shows he was discharged in March 1915 as medically unfit and died in May 1917. No CWGC record etc. found!
Subject is listed on both Beckenham War Memorial and Holy Trinity Beckenham War Memorial, I am in the process of adding details to these memorials

As home service only he did not have a medal record. His entitlement to CWGC would depend on whether his death was connected to his reason for discharge, which is highly likely because it was TB. For proof a death cert would be required and the claim can be made by yourself or via http://www.infromthecold.org/

Apologies, I meant to add that although most civil documents show his name as Herbert, he served as Henry Westlake!
